We truly value you and it is our greatest compliment when you refer those you care about to us for their services, products and quality care we are proud to provide. The Friends and Family Referral Program is our thank you for spreading the word about our team and our passion for serving the community.
Use our referral Share the Vision cards to share that you are referring a friend or family member!
It’s easy as:
- Grab a card and print your name on back.
- Pass it on to a family member or friend who will be a new patient.
- They bring it to the office and we record and thank you for your referral!

The Friends and Family Referral Program is not intended to reward the referral or recommendation of any item or service eligible to be reimbursed, in whole or in part, by Medicaid, Medicare, Tricare, or other federal or state healthcare programs (including the AHCCCS Managed Care and Medicare Advantage Plans). Accordingly, patients will not receive a reward for referring friends or family members if such individual receives service eligible to be reimbursed by one of these programs within 90 days of his/her service.
*Coupon card may NOT be used with any insurance-related service such as any applicable co-pays, co-insurance, or deductible amounts. Coupon card may only be applied towards cosmetic or "self pay" services.
