It pays to refer patients to the Arizona Eye Institute & Cosmetic Laser Center.

We truly appreciate every referral of a new patient we have never seen before.

Every new referral means a valuable gift to you!

But, we need your help!

In order to send you the gift, we need your full name.

Because it is so important to us to make sure you receive a gift for every referral you send us, we have developed referral cards to ensure we get your name with every referral patient you send our way.

Simply write your name on the back, and ask your friend or family member whom we have never seen before to bring the card with them to their new patient exam, or their appointment. If you have not received or been able to collect the cards, we are making them available online, to download and print.
